Anthropology Accession Number Request Form

This request form is used for an Anthropology field collection request. 

For all requests relating to Paleontology field collection, use this request form.

Before getting started with this form, you will need to have the following:

  • Valid Repository Number for the requested year
  • PDF version of permit or private land ownership letter (ensure you countersign the permit)
  • Legal authorization to obligate applicant organization

The year in the repository number should match the year collections were made.
The format of the number should be UMNH.R.YYYY.NN

Although the Utah Museum of Natural History is now also known as the Natural History Museum of Utah (NHMU), Collections & Research will continue to use the UMNH acronym for all activities.

 If applicant will be applying for external funding for project(s) under this agreement, it is important to include funds to purchase collections cabinetry for the housing of your specimens at UMNH. Please contact us if you need more information on specifics.
