Please attach a photo of the home school letter of intent for students. Home school letter of intents are filed with the school district stating that the parent/guardian takes sole responsibility for the child's education. Home school discount is only available to those with the letter. If you are from outside the state of UT, please attach a photo of your state's required documents.
